Community Nursing
At Grace Healthcare – Your Care Your Way, we have a professional team of registered nurses who are thoroughly trained to the very highest standard. Our team is prepared for and able to deliver outstanding nursing care with great care, empathy, and compassion with highest regard to professionalism and collaboration with you and/or your family. Some of the services we provide are:

- Day to day health supports in your home
- Pressure care and Wound management
- Diabetes management
- Complex disability care – PEG, continence, ventilator, bowel care
Daily Personal Activities
Our team at Grace Healthcare – Your Care Your Way will support you in your home/shared living environment with day-to-day activities of daily living to support with maximisation of your independence. Our team of disability support works can assist you with:
- Personal hygiene
- Showering, Bathing or Sponge bath
- Grooming
- Toileting
- Transferring you in and out of bed
- Assistance with meals
- Personal activities
- Life stage transition supports
Community Participation
At Grace Healthcare – Your Care Your Way, we believe in individuals living their best lives despite their disabilities. We support our clients to access community recreational activities individually or within a group setting or centre-based program which support you with developing life skills. To achieve this safely we ensure our team of disability support workers have a valid Driver’s license police check and valid qualifications.
Our community support services include:
- Attending to medical or GP appointments
- Attending school or other educational faculties
- Personal activities assistance such as going to movies, art classes and many more
- Leisure and recreation activities
- Shopping in your local centres or bill payments
- Support with maintaining your relationship within the community, contact with peers
- Safe use of public transport when travelling
- Innovative community participation – to help improve your wellbeing outcomes holistically
Household Tasks
Our disability support workers undertake household tasks such as cleaning services across all areas that our client regularly utilises within their own home or shared living environment.
- Vacuuming/Sweeping
- Mopping floors
- Wiping down benchtops, inside and outside of fridges
- Cleaning stove tops and microwaves
- Dishwashing
- Dusting
- Laundry – washing and drying and doing some essential ironing
- Bed linen changes according to your preference
- Cleaning bathroom and toilets
- And ensuring your home is left looking immaculate
Support Connection and Coordination
Grace Healthcare – Your Care Your Way is committed to ensuring your needs and goal are met to your satisfaction. Our support coordination team can support you to get the most out of your NDIS plan and achieve your goals through building your ability to connect with your informal, community and funded supports. Support coordination services can also empower you to live more independently through working with you to increase your capacity in maintaining relationships in your community, managing service delivery tasks using a mix of supports with your support coordinator.
